Registration INFORMaTION

Registration will be accepted on a first come, first serve basis, so early registration is strongly encouraged. You may either, mail payment with completed registration and medical release forms or scan-email completed forms and make payment later. BALANCE WILL BE DUE UPON ARRIVAL.

Confirmation letter with information packet will be sent out by email on receipt of registration and medical release form. The information packet includes Medical History Form, Photo release form (optional), full camp schedule, camp rules and map with directions to camp. The Player Profile Form for high school goalkeeper will be included in the confirmation packet for purpose of evaluation and recruitment.

All campers will be under constant supervision and may be expelled from camp if there are any disciplinary problems.

Medical Insurance / Medical Release & Emergency Information: When registering for camp, please fill out the current medical insurance information and sign on our medical release / parent-consent form with emergency information in case there is an emergency. Your daughter/son will not be allowed to participate if this is not signed.

The Hare Goalkeeper Academy does not provide medical insurance for campers. In the event of illness or injury requiring treatment, hospitalization, or surgery, family medical insurance must be used. Routine health care will be provided by our camp athletic trainer, however, staff will not dispense aspirin or other medications, unless instructed to do so. Although every attempt will be made to contact a parent or guardian in case of emergency, the medical treatment authorization / release portion of the registration form must be signed to grant permission for any medical attention needed to be authorized by the camp director or athletic trainer. If a player is injured, you may send a doctor’s note to us by mail or email to:

We must receive the doctor’s note prior to the start of the camp. We will then refund the full camp fee.

Medical History Form: Upon receipt of a camper registration and medical release forms, a confirmation packet, which includes a medical history form, will be sent out by email that needs to be completed by parent or a medical doctor. Physical exams conducted as of September for the current school year may be attached to our Medical Form provided all necessary information is provided. It is imperative that you remember to bring your medical history form to camp. Please note that a camper cannot participate without a completed medical history form and medical release & parent consent form.

Player Profile Form: The Player Profile Form is for the high school goalkeepers in the GK ID Academy Camp or GK Residential Academy Camp at Westminster College in conjunction with the College Bound ID Camp and the Titan Residential & Commuter Soccer Camp. This form will be provided for purposes of evaluation and recruitment. Upon receipt of a camper registration and medical release forms, a player profile along with other forms in the packet will be sent by email that needs to be completed by GK ID or Residential Academy Camp participant or parent.

IMPORTANT RULES: No tobacco, alcohol or drugs of any kind are allowed at all Hare Goalkeeper Academy Camps. Any violation of this rule will result in immediate removal from camp at the parent's expense. Also, the campers are responsible for any damages they might cause to any camp facilities or equipment, except when on the field during training. Thank you for your understanding.
